A New Claim Form has been introduces by the Fund Office!
The Fund Office requires a new Claim Form every six months or with each new injury or accident. A Claim Form is necessary for the Fund to determine eligiblity for benefits. Processing of claims may be delayed due to lack of information. <GET NEW CLAIM FORM >
UPDATE YOUR
ADDRESSES
It is
essential that the Fund Office always has your correct address.
For instance, we are unable to process interim Vacation &
Holiday withdrawals if the address on your request is different
from the address in our records. If you need a change of address
form, you can request one from Member Services at 800-595-7473,

HOLIDAYS
The Fund Office will be closed for Veteran's Day on November 11th, Thanksgiving on November 27th & 28th, Christmas on December 24th & 25th, New Years' on December 31st & January 1st, 2009 and President's Day on February 16th, 2009.
