Notice to Pensioners

For a Pensioner to continue receiving your monthly pension benefit payments under the Rules and Regulations of the Plan, you be retired and refrain from work which is prohibited under the Plan. If you return to work in prohibited employment, your pension benefit will be suspended.

The way that the Fund ensures that you continue to receive your monthly benefit payments without interruption is to require that each year by May 15, pensioners certify that they are not engaged in prohibited employment.

There are two ways you can do this:

1. You can complete and return the Annual Statement by Pensioner to the Fund by May 15, 2008. If you return that form, you do not need to mail any tax forms to the fund office. Please note, however, that the Annual Satement by a Pensioner must be signed in the presence of (1) a Notary Public, or (2) your Local Union Busines Manager in DC#16. If you live outside California, you must have the form notarized.

OR

2. You can file a copy of your W-2 form and a copy of your Federal Income Tax Return with the Fund by May 15, 2008.

If you do not forward either the completed Annual Statement by Pensioner or the tax forms described above to the Trust Fund by May 15, 2008, your pension benefits will be suspended beginning July 1, 2008 and your benefits will not be reinstated until the Fund receives the appropriate verification.
