Beginning April 1, 2012, SCPT Participants and eligible spouses, have access to an enhanced Wellness Rewards program. You can earn up to $100* for completing 3 simple activities.

Request a free biometric screening kit from Summit Health.

Follow the instructions included in the kit to complete your screening. Return the test in the pre-paid envelope and receive your screening results in 10 to 14 days. The Fund bears the cost of the kit once the order is placed. Please complete the screening and return the kit for your results.

Complete a Wellness Assessment by going to "Healthy Lifestyle Rewards".

Log in, or follow the instructions to create a new account, and click on "Wellness Assessment". Complete the online questionnaire with information about your personal medical history, lifestyle and health risk screening results.

After completing the assessment, you will receive a Wellness Score and a personalized report with tips on how you can modify your lifestyle to improve your health.

Completing steps 1 and 2 earns you a $75 reward.

Please do not discard your Visa card as future rewards will be automatically applied. There is a fee for replacement cards.

To start a program, go to "Healthy Lifestyle Rewards". Then, log on to Healthy Lifestyle Rewards, select "Health Programs" and the program that interests you. To enroll in a program, click on "Enroll in Full Program" at the bottom of the page.

Completing a Healthy Lifestyle program earns you a $25 reward.

*Although dependents who are 18 or older have access to the Healthy Lifestyle Rewards content, only Southern California Pipe Trades participants and their spouses are eligible for rewards.
