Once you’re eligible for benefits, you will be sent a welcome package that includes enrollment materials to complete and return to the Fund Office. You can also download these fillable forms on the Forms page. Then print and submit properly completed forms to the Fund Office by email, fax or mail.
Important Note: You must complete the enrollment materials to enroll eligible dependents or they won’t have coverage.
