The only way to update your Plan address is to send a completed Change of Address Form to the Fund Office as soon as possible after any change. The easiest way is to download and fill out the form online, print, complete and sign it and send it to the Fund Office by email, fax or mail. Don’t delay or you may miss benefit statements and important updates.
