Beneficiary Form

You may make or change a beneficiary designation at any time by completing, before the date of your death, a properly completed Enrollment & Beneficiary Form. The beneficiary designation takes effect when the signed form is received by the Fund Office. The Hartford Life Insurance benefits have a separate beneficiary designation form that must be completed upon enrollment. 

If you do not designate a beneficiary or if the beneficiary predeceases you, the Plan pays benefits in this order: 

  1. To your surviving lawful spouse or domestic partner
  2. If none, divided equally among your surviving children, including legally adopted children 
  3. If none, divided equally to your surviving parents 
  4. If none, divided equally among your surviving siblings
  5. If none, to your estate

Your Enrollment & Beneficiary Form names your beneficiary under the Health & Welfare Fund, Defined Contribution Pension Fund and the SCPT Defined Contribution Fund.
