Life Insurance Benefits

If you die while covered, your beneficiaries may receive a life insurance benefit. 

This group term life insurance provided by your employer gives extra protection that you and your survivors may need in the event of your untimely death. A benefit is provided to your beneficiaries in a lump sum following your death while covered.

Eligibility

You are eligible for life insurance benefits if, at the time of your death, you have active eligibility under the Landscape, Irrigation & Lawn Sprinkler Industry Health and Welfare Plan .

These life insurance benefits are provided through your Plan’s group insurance policy with The Hartford Financial Services Group, Inc. Your employer pays 100% of the premiums for your coverage. To learn more about your life insurance, visit www.thehartford.com/employee-benefits. You can register and log into your account to see details about your benefit and view or download a certificate of insurance. You may also request a copy from the Fund Office.

What You Must Do

The Fund Office will automatically enroll you for coverage when you become eligible. However, you must designate your beneficiary as soon as possible when coverage begins.

  1. Make sure you have a properly completed Life Insurance Beneficiary Designation Form on file at the Fund Office. If you die while covered, the beneficiaries you name receive payment from the plan. Designations or changes are not accepted by the Fund Office on or after the date of death.
  2. Make sure your survivors contact the Fund Office within 60 days of your death. 
  3. Your beneficiaries must submit a certified copy of the death certificate and request the death benefit in writing within 12 months after your date of death. Death benefits are not paid unless all supporting documents are received by the Fund Office within the 12-month timeframe.
